Summary of Changes from Previous Version

May 2020 – This guideline has been rescinded and the provisions subsumed in the Work Health and Safety Reporting Procedure.


The Unit Head field has been updated from the Director, Workplace Relations to the Director Culture, Capability and Wellbeing to align with current structure.


September 2019 – all references to the incident and hazard reporting system SCRIM have been updated to Protecht.


This is the first version of this Policy, as published under the approval of the Executive Director, Human Resources in accordance with Delegation GOV10A.
